Simple virilizing is primarily caused by mutations in the CYP21A2 gene, which leads to a deficiency in the enzyme 21-hydroxylase. This enzyme deficiency disrupts the production of cortisol and aldosterone, causing the adrenal glands to produce excess androgens. The condition is inherited in an autosomal recessive manner, meaning a child must inherit two defective genes (one from each parent) to be affected.
