The true conjugate cannot be measured directly because the sacral promontory is not accessible externally. Therefore, it is often estimated using other pelvic measurements. One common method involves measuring the diagonal conjugate (the distance from the lower border of the symphysis pubis to the sacral promontory) and subtracting approximately 1.5 to 2 cm.
